Handbook Signature Form I have received a copy of the Springdale Public Schools District Policies Handbook and a copy of the Springdale High School Student Handbook., which includes policies on attendance and discipline, computer use, internet use, etc. Student Work or pictures on World Wide Web Before your child’s photo writing, or artwork can be displayed on the school or district website, we must have parental permission. Because once material is posted, it can be seen by people all over the world, we request that parents sign this permission form. Before any student’s work or picture is placed online the teacher responsible checks the file for parent permission. For security reasons, only first names will be used and work will appear with a copyright notice prohibiting the copying of such work without express permission. In the event requests such permission those requests will be forwarded to the parents. No home address or telephone number will appear with such work. A copy of the picture or material is available. If you have any questions or wish to request the removal of a student picture or school work, please contact the school office. Parental Permission I understand my child’s photo, writing, or artwork may be published on the World Wide Web in accordance with the Springdale Public Schools’ web standards at http://www.sdale.org/technology/Webstandards My child’s work or photo will be removed immediately upon my request. This form is valid for the entire school year associated with the date stated above. Acceptable Use Policy (AUP) After reading the Acceptable Use Policy (AUP) we understand that technology access is designed for educational purposes. I will abide by the stated terms and conditions set forth in the AUP. It is further understood that violations of the regulations are unethical and may constitute disciplinary actions including revocation of access to technology, termination of employment, of employment, or legal actions in the case of criminal activities. Supports and reflects School Board Policy Code CBCBC/GDPDD. (Students will be unable to access computer or the internet in any location in the school setting.) Family Educational Rights and Privacy Act (FERPA) Parents or eligible students have the right to inspect and review the student’s education records maintained by the school. Schools are not required to provide copies of records unless, for reasons such as great distance, it is impossible for parents or eligible students to review the records. Schools may charge a fee for copies. Generally schools must have written permission from the parent or eligible student in order to release any information from a student’s education records, without consent, to the following parties or under the following conditions (34CFR 99.31).
